Website & Forms
Our public website does not require you to submit PHI. Please do not include sensitive medical details in general contact forms. When PHI is requested, we use secure, access-controlled processes.
Complaints & Questions
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health & Human Services. We will not retaliate against you for filing a complaint.
